Transition Timeline

Transition planning encompasses a wide variety of elements including education, employment and independent living. Knowing when to start planning and different milestones is key to ensuring all aspects of transition planning are covered. The Transition Timeline identifies the activities and actions to consider for the core transition years ages fourteen to twenty.

  • Inform Area Agency of anticipated specific adult services support needs.

  • Revise the transition plan.

  • Build on part time paid or volunteer work skills and opportunities.

  • Consider a Vocational Evaluation through the school system to update skills obtained and current interests, as well as areas that need work.

  • Discuss priority status on the Wait List.
