Transition Timeline

Transition planning encompasses a wide variety of elements including education, employment and independent living. Knowing when to start planning and different milestones is key to ensuring all aspects of transition planning are covered. The Transition Timeline identifies the activities and actions to consider for the core transition years ages fourteen to twenty.

  • Learn about Social Security and Medicaid benefits.

  • Discuss post secondary education options with the team and guidance office.
