Transition Timeline

Transition planning encompasses a wide variety of elements including education, employment and independent living. Knowing when to start planning and different milestones is key to ensuring all aspects of transition planning are covered. The Transition Timeline identifies the activities and actions to consider for the core transition years ages fourteen to twenty.

  • Connect with your local Area Agency, if you have not done this yet.

  • Identify personal learning styles and the necessary accommodations for learning and working.

  • Request transition information from your school and Area Agency.

  • Discuss graduation options (Certificate of Completion/Diploma) and request information on your school’s policies on graduation.
