The IEP Transition Requirements Resource (I-13)

The IEP Transition Resource is a multimedia training and reference tool designed to increase understanding about IEP transition requirements and indicator 13. This is a series of 18 short videos and a listing of resources on transition services and Indicator 13.

This website and resources were developed for special educators and trainers in New Hampshire to assist them in understanding transition IEP requirements and provide multiple ways for them to learn about the requirements through video, online links to resources, and connections to the NHSEIS IEP system. The tool is designed to enhance training efforts that are currently underway by the NH Transition Community of Practice.

While the tool was designed for special educators and trainers, it also can be used by any transition stakeholder (e.g., families, agency staff, teacher education faculty) to learn about the transition requirements that need to be addressed in special education programs.  Learn more from the IEP Transition Resources.

Meet Morgan, a talented student with a dream and a Plan to get there.

Morgan is a busy high school student with a fondness for soccer and softball. She is a member of the student council, holds down a part-time job and has her eye on a career in the medical field. And by the way, Morgan has a learning disability, which hasn’t gotten in her way!

With the support of her family and school, Morgan has developed a Transition Plan that will prepare her for life after high-school, including college, preparation for the career field of her choice, and an independent life.

“My goal after I graduate from high school is to attend a four-year college and enter the medical field. I really enjoy the
idea of helping people lead healthy lives. During my high school years, I have been active and like spending time outdoors.
I play soccer in the fall and softball in the spring and I’m also a member of my high school’s Fish and Game Club. My days are
pretty busy so the most fun part of my day is actually soccer practice after school because it’s a way for me to relax.”
